We are offering a great opportunity to join our Sheffield Dementia Advice team, providing personalised support to people diagnosed with dementia and their carers.
This is a newly created role to join our dynamic Sheffield local services team, working to increase reach in 9 primary care networks across the city. The post will be working in collaboration with PCN (primary care network) staff, GP’s, Admiral nurses and other health and social care professionals working in dementia diagnosis and care.
This role requires travel for home visits and meetings within the Sheffield area, mileage is payable within the border. Part of this role will be based at home, this is when you will complete admin tasks.
The service provides person centred and outcome focused information, advice and support. This is provided in person on an individual basis, over the telephone, via virtual options, or in writing as well as through in person and online group presentations. You must therefore be comfortable presenting to groups of people.

- Have a good understanding of professional boundaries, the importance of a strength-based approach and outcome based work
- Have some knowledge of dementia and the challenges and barriers faced by those affected by it
- Have confidence to develop relationships and networking internally and externally with professionals and organisations in Swindon.
- Be empathetic and non-judgmental in your approach, committed to equal opportunity.
- Have a sound knowledge of IT systems such as Microsoft Office and Salesforce or be commitment to learn.
