We are looking for a warm, thoughtful and organised Dementia Adviser to join our supportive Worcestershire team, working 21 hours per week across the West Region (Covering Wyre Forest area) on a permanent basis.

We're launching a new county-wide dementia support service across Worcestershire in partnership with Herefordshire and Worcestershire Health and Care NHS Trust, and we're looking for Dementia Advisers to join us at the start of this exciting journey. This is a community based role covering Covering Wyre Forest area. You will spend much of your time out in local communities, working in GP Practices meeting people face to face, carrying out home visits, attending community groups and building relationships with local organisations. While some administrative work can be completed from home, this role is centred on being present in the community and supporting people where they are.
As a Dementia Adviser, you'll support people from the point of diagnosis and throughout their dementia journey. You'll take time to understand what matters to each person, provide information and guidance, and help them access the support they need to live as well as possible.
Working closely with GP practices, community groups, healthcare professionals and local partners, you'll help ensure people affected by dementia receive the right support at the right time.
This is a unique opportunity to help shape a service from the very beginning. You'll build trusted relationships in your local community, contribute ideas, and play an important role in developing support that makes a lasting difference to people's lives.
You'll be joining a newly commissioned service covering all Worcestershire Primary Care Network (PCN) areas and working alongside NHS colleagues to develop strong, effective ways of supporting people affected by dementia. From day one, you'll have the opportunity to influence how the service grows and help shape the future of dementia support across Worcestershire.

Please note that regular travel across Covering Wyre Forest area is required. You will need to be able to travel independently between community locations where public transport may not always be available.
About You
We're looking for compassionate, motivated people who share our commitment to supporting those affected by dementia and helping them live well. You will have:
- An understanding of dementia, or a willingness to learn about the experiences of people living with dementia and the people who support them.
- Experience providing information, guidance, advice or emotional support. This could come from health, social care, community, voluntary, customer service or other people-focused roles.
- Strong listening and communication skills, with the ability to build trust and positive relationships with people from a wide range of backgrounds.
- The ability to organise and manage your workload, balance priorities and use support when needed.
- Confidence using IT systems to keep accurate and confidential records.
- A compassionate, respectful and non-judgemental approach, recognising and valuing people's different experiences, backgrounds and circumstances.
- The ability to travel independently across your local area.
- Confidence communicating with a wide range of people, including healthcare professionals, community partners and members of the public. You'll be able to adapt your approach to meet different needs and may occasionally speak at local events, providing information about dementia and helping people access further support.

Key Responsibilities:
- Build supportive, trusting relationships with people living with dementia and those who care for them, recognising and respecting their individual circumstances, needs and wishes.
- Help people maintain their independence, improve their wellbeing and feel more confident and supported in their day-to-day lives.
- Provide tailored information, advice and emotional support that reflects each person's background, culture, values and preferences.
- Support people in ways that work best for them, including over the phone, in person and alongside other services and professionals. This may include home visits where appropriate.
- Take part in the referral and telephone support rota, responding to new referrals promptly and sensitively to ensure people receive support as early as possible.
- Manage your own workload, maintaining accurate, confidential and respectful records.
- Help people access local services, community activities and support networks that meet their needs and reduce social isolation.
- Work closely with colleagues, sharing ideas and contributing to the ongoing development and improvement of the service.
- Facilitate peer support groups, creating welcoming and inclusive spaces where people affected by dementia can share experiences, build connections and access information and support.
- Host drop-in sessions, attend community events and provide information about dementia and the support available locally.
