About The Role

We have an exciting opportunity to join our team as a Dementia Adviser, working within our alliance with the Brighton and Hove Memory Assessment Service.

This alliance brings together the NHS and the Alzheimer’s Society, as a Dementia Adviser within the service you will work as part of the multi-disciplinary team to deliver dementia support, signposting, and advice across the city.

This is a role that allows you to make a real difference in the lives of those affected by dementia, pre diagnosis and post diagnosis.

About you

  • Be an empathetic listener, able to assess and evaluate client need, using a non-judgemental manner, a person centred and empowering approach.
  • Be able identify, signpost and refer to services needed as a result of your assessment and that of the other professionals in the team.
  • Able to work under pressure and work within Memory Assessment Services appointment system and processes.
  • Self-motivated and able work independently from home using your own initiative, but equally you will be a team player, keen to work with, and support your colleagues.
  • You have some knowledge or personal experience of dementia and the challenges people affected by it may face.
  • Knowledge of relevant legislation such as the Mental Capacity Act, Safeguarding Adults, Data Protection.
  • Experience of record keeping and using client record systems and the importance of adhering to procedures and policies.
  • Good IT skills.
  • Able and confident to travel within the service area and able to attend clinical settings and visit people in their homes.
  • Able and confident to represent the needs of people affected by dementia at community events and meetings.

     

    Interviews will take place on the 29th May.

    What you’ll focus on:

  • Provide pre and post diagnosis support within the Memory Assessment service in Brighton and Hove.
  • Work with the whole Memory Assessment team and with other professionals, such as GP’s, and other health and social care services.
  • provide a person-centred and outcomes focused information, advice and support service to people affected by dementia and their carers.
  • You will identify their needs and the services they may wish to access, give advice about how to live well with dementia and support the improvement of their sense of wellbeing.
  • Support may be provided in person, over the telephone, via virtual options or in writing.
  • be working from home, at the Memory Assessment Service, in clinic and completing home visits.
  • aim to reach people affected by dementia from all communities and work to make your service as inclusive as possible.
